what to put on appraisal review form?please help!?

hi everyone, i was given a form " mid-year review form", its my first time to have this and i dont have any idea on what to put. there' s a part that says " job holders comment" i want to know what should i put there, should i put the reason why i deserve a salary increase? or should i put my achievements? please help, if possible please give me a sample letter or format. thanks you so much. your reply/help will be much appreaciated.
