How much does T-Mobile charge upfront?

I want a Unlimited Data, Text, and Talk with the iPhone 5 which is $0 upfront , but $10 upfront for the sim kit. Now I'm a little confused. Do I just pay $10 that day and the phone is activated and good to go and in a month, I pay the phone bill or do I pay the phone bill and the sim kit that day?
