Turtle question, help please?

My 5 year old yellow bellied slider was fine yesterday. Healthy and happy. I fed him later on and I noticed today that he hadn't eaten it. He was laying around all day on his platform and I just went to check on him. I picked him up and his neck rolled to one side. I think he's gone :( What did I do wrong?
