Questions about the Medal of Honor?

It is awarded for going "above and beyond the call of duty" which means doing something heroic you weren't necessarily ordered to. Does this mean that if a cook were to go save someone's life while getting shot at might be awarded one, whereas a pararescueman doing the same job wouldn't?
