how to do gaming videos?

I was just wondering a few things about doing this, I know how the set up works and that I need a capture card but I was mostly wondering how people use headphones while doing them. I realize the cameras and/or sound equipment is external but when a capture card is connected how do peoples voices get picked up online into the video?
